In order to register for a class, you must fill out the following form. Attendance confirmation is based upon availability. Once we receive your registration, we will notify you within 72 hours if you are confirmed for admittance. Class size is limited to 24, so please signup early. If a class is full when we receive your registration, we will place you on a “wait” list, and notify you of such.

In-clinic Ultrasound Training

Notes will be in a digital format and will be sent to the email you provide. Please check for them 72 hours ahead of the course to ensure receipt. It is recommended you download the digital notes to a laptop or tablet that you bring with you. No written notes will be provided.

Note: A $200 deposit is required to guarantee admittance into the live classes in Texas. Full payment is due 30 days prior to the class date. If you would like to go ahead and pay your full class tuition now, you may do so.

Special Requests and Comments: Lunch will be provided, so please let us know if you have dietary requirements.

Very Important Information:

Click here for hotel info. Please reserve your hotel room when you register for a class. We have a guaranteed rate at the DoubleTree by Hilton Farmers Branch, but not a guaranteed block of rooms. It is on a first come/first served basis. Please reserve early, in order to stay at our hotel of choice, which offers many amenities.

Policies

Confirmation is based on a first come/first served basis. Class size is limited to 24, so please register early. We will place you on a wait list if the class is full when we receive your registration. If wait listed, we will contact you if we receive a cancellation, and can confirm you in the course.

All confirmed registrations are NON-REFUNDABLE. Cancellations received less than 30 days prior to the course date, or no shows, will result in a full class charge to the credit card on file. Non-refundable course fees will not be applied towards other courses. You may cancel without penalty up to 30 days prior to the class date. Your registration is your acceptance of the above policies.
